Lean body mass (LBM) is total body weight minus fat mass — it includes muscle, bone, organs, connective tissue, and water. LBM is used in sports science to tailor protein intake recommendations (often expressed per kg LBM), calculate drug dosages, and track changes in muscle mass independent of body fat fluctuations. During a cut, monitoring LBM helps confirm you are losing fat rather than muscle. During a bulk, rising LBM with controlled fat gain confirms the surplus is being used effectively. LBM is also a stronger correlate of metabolic rate than total body weight.

How to interpret your results

Compare your LBM over time — not in isolation. At the start of a fat-loss phase, record your LBM as a baseline. After 4–6 weeks, if total weight has dropped but LBM has held within 0.5–1 kg, you are losing predominantly fat. If LBM has dropped by more than 1–2 kg, your protein intake or calorie deficit needs adjustment. During a muscle-building phase, a rising LBM alongside modest fat gain confirms the training stimulus and calorie surplus are working. Stagnant LBM despite a surplus and consistent training usually points to insufficient protein, poor sleep, or inadequate progressive overload.

The science and formula

LBM is typically estimated from body fat percentage: LBM = total weight × (1 − body fat % / 100). Without a direct body fat measurement, formulas such as Boer (1984) and James (1976) estimate LBM from height and weight alone. Boer formula for men: LBM = (0.407 × weight in kg) + (0.267 × height in cm) − 19.2. For women: LBM = (0.252 × weight) + (0.473 × height) − 48.3. These are less accurate than body fat percentage-derived estimates but useful when a body fat measurement is unavailable. DEXA scanning remains the gold standard for precise LBM measurement, offering compartmental analysis of muscle, bone, and fat mass separately.

Important limitations

LBM calculations are only as accurate as the underlying body fat measurement. Errors in body fat percentage (which can be ±3–5% depending on method) translate directly into LBM errors. The body fat-derived LBM formula also treats all non-fat tissue as equivalent, but bone density varies significantly — athletes and postmenopausal women can have markedly different bone mineral content at the same height and weight. LBM naturally fluctuates by 1–2 kg daily due to glycogen and water changes — particularly after a high-carbohydrate meal or following intense training. Always compare LBM trends over weeks, not day-to-day readings.

What is the difference between lean mass and muscle mass?

Why does protein intake get calculated per kg of lean mass?

Protein requirements are driven by muscle tissue maintenance and growth, not by fat mass. Fat cells do not consume protein. Calculating per kg LBM (or per kg bodyweight for simplicity) gives a more physiologically accurate target, especially for leaner individuals.

Does LBM affect basal metabolic rate?

Yes. LBM is the primary driver of BMR — skeletal muscle and organs are metabolically active tissues that burn calories at rest, whereas fat tissue is relatively inert. Each additional kg of LBM raises BMR by approximately 13–20 kcal per day, which compounds meaningfully over time with sustained muscle gain.

Can I gain LBM while losing fat simultaneously?

Body recomposition (simultaneous fat loss and muscle gain) is achievable for beginners, those returning to training after a break, and overweight individuals in a moderate deficit. For trained athletes with low body fat, true recomposition is slow and difficult — dedicated bulk and cut phases are more time-efficient for advanced trainees.
